Security issues with Custom installation and later patching?

If a Custom installation of Office is done initially (i.e., less than full
product features installed), then a Hotfix or SP is subsequently
downloaded/installed, will the Hotfix or SP install ALL feature updates, even
for those compnents not initially installed?

My issue is this:
If, after a Hotfix or SP is installed, a user subsequently enables
additional features by pointing to a shared .msi file (which was used for the
initial installation), will those features be patched as a result of the
previous Hotfixes or SPs or could a security vulnerability be unwittingly
created?
