Seeking Correction to "Tell a Friend" Script

I have a site up: http://eachdaymarketingandgraphics.com that I created with
Publisher 2003. I would like to include a "Tell a Friend: option. As you
can see on the Services page, when visitors clicked the link, they are sent
to a page where they can include their name, their e-mail address, the
friend's e-mail address and a message. I got this script from asp101.com.
But, as you can see, it doesn't allow you to enter a message and when you
click on Send Message, it goes to an error page.

Can anyone assist me with the code on this page?

Sorry, but when I look at
http://eachdaymarketingandgraphics.com/index_files/Page390.htm
the script is sitting on top of the page. It appears that you did not use
the insert html code fragment tool to insert it into the page.

I would suggest that you go back to the site where you found the script, and
see if there are any instructions. In general I would suggest that if you
have problems with one code snippet, then google for another. There are a
zillion free code snippets out there, and if I can't get one to work, then I
just try another. That has been faster, more time efficient than trying to
troubleshoot code.
