select different forms

My database has multiple user entry forms, all similar but all have
different fields, the form is selected by the user choosing the 'referral
source'. I'd like to improve the form selection screen, so that the user
can read a list of referral codes and enter the number that corresponds to
the source, and a command button then takes the user to that form (There's a
button on the form that saves the record, closes the form and re-opens the
selection form). I think I could do this with a drop down - but I'd prefer
to do it this way. Any help appreciated.
