Send a reminder letter

I have a list of vendors in a table with their insurance expiry date.

I have a query that lists the vendor who's insurance is about to
expire; 30 days before the date.

I created a form based on that query that shows the vendor, expiry
date and a button to create a report.

Now I want to change some verbiage around in the report..instead of
saying its about to expire I want to send a notification saying the
insurance info is wrong or missing etc. (use a combo box to change the
wording)

I also want to chose the date I sent the letter, I don't want to be
obliged to use today's date.

I want to store all that information in a SEPARATE table so I can
refer back to it.

You can certainly do what you want. If you could be specific regarding the
area in which you are encountering problems, it's likely that someone will
be able to make useful suggestions. Otherwise, you need to begin by making
it clear just what question you are asking and what help you need.
