Send form data to database

I'm trying to create a form so users can enter a few fields of info and
submit the data to a SQL Server Database.

The site I'm working with is a company intranet created using Sharepoint
Services. I currently don't have the option to send to database (it's greyed
out).

How can I setup a form on this site to publish form data to our SQL server
database? Thank you so much to whoever can help.

Do you have a web open?

Have you set the 'compatibility' under "Authoring" correctly? (Page Options
on Tools menu). You may need to experiment with those settings.
