Send Option When Creating Form in Outlook 2003

Hello,
Does anyone know how to setup a form to automatically send the form to a
specific e-mail address when clicking the send button.
I've created a button and set the value to send to an address, but when
clicked, it brings up the "Select Address" window with the address in the
"To:" and "CC:" field.

One easy method is to pre-address the form as part of the design. Explain
how you have "set the value to send to an address".

Thank you for your response Sue...I think I figured it out.

I do have another question though, if you wouldn't mind.
I'm trying to make my for read only in certain fields, but only on certain
fields...not read only from the originator, but by the recipient. Any clues?

You can set the appropriate controls on the read layout of the form to
read-only.
