sending blank contact form

i would like to be able to send the contact form or template as an email so
i can include all of the information into my contact manager. i know of the
office acces form but i want it to look like the outlook contact form when i
email it and when they reply it will come back to me in the same manner

That's not going to be possible, because of Outlook forms security.

can i send a contact with just the name filled in as a attachment and they
can fill in the rest of the form and email it back

No, it's not that easy. They'd have to save it as a file and then attach it
or use the Forward as vCard command.

And that all assumes that the other person has Outlook and not some other
mail program and that he or she is willing to go through whatever
instructions you provide.

Plaxo is supposed to be able to do that, but it can cause occasional issues
with Outlook.
