Sending Outlook Forms (2003)

I have designed and pasted to edit page and I've even unchecked the Separate
Read Page choice (per post on 12/3/2008) but it still sends a blank email
when I try to send the form. What am I missing? Please help. Thanks in
advance.

P.S. - I have also published the form

Trying to send a custom form over the Internet isn't usually a good idea and
rarely works as expected. Are you sending in RTF format?

I let it save as it was, which I think is the IPM.message format and I
published it in the fdm format (per our IT guy).

So just so I'm clear, outlook forms are only to be created and utilised by
me and not forwarded to anyone else using outlook?

You have to publish the form with a custom message class, in this case one
derived from IPM.Note. You don't publish a form in fdm format, you publish
it from an open form in design mode.
