Sending to specific recipients, field populates with time/date?

I have created a form and used the "To..." button to make one big
button that, when clicked, is sent to three members of my team.
Unfortunately, it is not working that way. I set the initial value of
the field to <my email address>, but when the form is ran (both as an
..oft file and as a published form) it autopopulates as time and date
and brings up the address selection book. I am using Outlook 2003, has
anyone ran across this? I am brand new at form creation and i am sure I
am just missing it somehow.

What's the exact formula (with the address munged, of course)?

Could you explain a bit more clearly what this "one big button" does?

As I am not sure what you mean by "formula", I am probably in over my
head. I basically created a "To.." button that the caption reads
"Click here to send". In the value tab I set the initial value of the
field to <my address>. This is all shown when I right click.
I have a feeling you are slapping your head and groaning at my
inexpertise....

What you set was a formula for the initial value. Obviously, you didn't set it literally to <my address>. We need to see the literal and complete text of the formula. Copy and paste it into your response, then edit it as necessary to avoid exposing your real email address.
