Server indicates files are saving when they are not...

All,
A user made changes to a project plan. Saved the file to the Server and
Checked the file in to the server. The Queue indicates the Save is 100%
Successful and the Check-In is 100% Successful. When the file is opened
again, the changes were not saved. No errors in the queue, no errors on the
Event Log. The User then downloaded the file to their machine and made
changes, then uploaded back to the server. Changes were saved. But, then
the text field changes were some how lost. What could be happening here?
Help.

What's in the queue. Worth restarting it. Also, are you sure you user is
connected to the right instance?
