Service request form

Hello,

I want to create a simple form in outlook to use as a service request form
for facilities maintenance requests. I have Microsoft office infopath and
there is a perfect form with pull down menus etc...

1) Is there a downloadable form for service requests?
2) can forms in infopath be exported to outlook?

If the answer to both of the above is no...then can someone direct me to
instructions for creating the form and publishing so that all users can
access?
