Setting Address Info for Envelopes

How do I set what information I want to come up when I'm
printing a contact's address (from Outlook) in MS Word?
For example, it brings up the person's name, but not
their title (e.g. Mr.). Also, can I get it not to
say "United States of America" after the address?
