Setting an autoid

How do I set a value for an ID, where each patient has a separate set of records (ie, Surgery1, Surgery2, Surgery3, each are auto labeled as 1, 2, 3 respectively for patient A) and patients B, C, and D have Surgery1, Surgery2, Surgery3 for each of their respective group sets
