Setting Bookmarks but not taking up space

I have a document and I modified it with Bookmarks. However, the layout of
the document changed because of the space (rows) used by the bookmark
definitions; e.g., { SET MyName "FRED" }. Is there a way to get the space
back?

Suzanne - I did as you said. The bookmark definitions disappear but I am
left with a blank row for each bookmark. For example;
Start














{ASK FirstName \* FirstCap "What is the first name of the patient?}


{ASK LastName \* FirstCap "What is the Last name of the patient?}

End



When Alt+F9 I get

Start





End

So the insertion of the bookmark definitions is affecting my print layout,
print preview, etc.

What I did to get around this was put the bookmark definitions on the first
page of the document. But this does not seem like the best idea since it
screws up the page numbering etal.

What am I doing wrong?



Secondly,

Lets say I want to email a document to a patient. I just want him to see
the bookmarks applied to the document, not their definitions. How do I
create a view-only document as an attachment or must I just do File->Send
To -> Mail Recipient?
