Setting form properties

I have a form www.blackcreekbaptistchurch.com/visitor_feedback.html The
problem I am having is getting the connection to work. I have a sql data base
with www.hostmonster.com named blackcre_FBCBCdata1. I set the connection
network with server, server type = sql server, server name= loacl host,
database name= blackcre_FBCBCdata1. I set the unsername and password. When
I try to select page for confrmation it will not let me select the
visitor_feedback.html file. When I type it in manually and click ok I geta
error (something I'm getting used to with FP) that says I need to create a
table HOW? Also it says something about an agrument. (I have enough of those
with my wfe).

note: my server has fromt page extenisions and some how I did manage to get
the results e-mailed from my server. But I cannot set confirmation page url.

Whats up?
