Setting up fill-in headers on a multi-page fill-in form

i am new to the developer tools in Word and I'm developing a fill-in form for
others to use. I want to be able to have a running heading that the user
fills in fields in. Word 2007 does not allow you to directlty place content
control fields in headers, footers, etc. I know there is a way around this,
but I haven't found it yet. I am sure that somone has done this before. If
you have, I would really appreciate your help. Thanks!

Word 2007 DOES allow you to place Content Controls in the headers and
footers.
