Short Leave application form template

I am working on a construction site. There are total 60 workers working on my
site. To keep better control, I need every employee, who leaves the site for
a very short duration ( say, 2 Hrs ), will write a short leave application
form.

I am looking a suitable template for it.

This is pretty specialized. You'll probably need to create your own,
although you might be able to modify the Absence Request Form or Employee
Absence Tracker or something here:
