show some columns once on form to be included in each row in detai

forgive me for being naive but i only found infopath yesterday! I'm
connecting to a sql db and have created the form based on a table in the db.
eg of table columns: branch,month,complaint date, complaint name etc
I'm collecting info from each branch to merge in the db
I want the form to look something like this

BRANCH:____________ MONTH: ______________

complaint date:__________ complaint Name:___________ .....etc
complaint date: __________ complaint Name:___________.....etc
.....etc

I hope this is clear! Please could someone tell me how to do this.

My initial thought is to use a combination of Master/Detail and repeating
tables. The Branch and Month could be at the master level with a repeating
table displaying the detail of the complaints.
