Sign up webpage

[heal my pc!]

I'm trying to set up a webpage for people to add their contact information to
add to a customer database - just a spread sheet or a text file. It's just
going to run on the counter top for my customers to add their info, if they
want to receive coupons and such.

However, I have no idea how to do this and the searches in help and
templates haven't born any fruit. Does anyone know of any templates that
will allow me to create something like this?
