Usually such forms require that you print the form anyway. If you do have to
send it in electronically then you can either
* print the form, sign it and scan the entire form back
* scan your signature and paste it into the document
* use a pen-enabled system such as a drawing pad, or a Tablet PC either
directly with the software, or write into a paint/ink program and paste the
result into the document
Unfortunately you haven't supplied any details of what software you are
using so I can't be more precise.
