Signature turns off by itself

I have a user whose signature in outlook turns off by itself.
I went into outlook myself and enabled the signature. a week later, it has
been set to none. I know the user is not changing it as they dont know where
to go to enable it in the first place. Any idea as to how this signature
turns off by itself?

Does this happen on new mail created from Outlook or when using the Send to
option in any office application?
