simple check box with some protected fields

I'm trying to set up a simple form !!!! with some fields
protected and some fields for a check box of either yes
or no.

Hi Vince,

You can do this with formfiels and the procedure in the article "Making
groups of Check Box Form Fields mutually exclusive (so that they behave like
radio buttons)" at:

to make the checkboxes mutually exclusive. If might be simpler to use
DropDown type formfields containing just a Yes and a No and the items that
can be selected. You would not then need to use any code.
