Sliding Scale

Is there a way to add a sliding scale into a word template? I have a
template where I want people to select a 1-5 rating.

I need the template to be fully editable so I cannot protect the
document/form.

There's a method at http://www.gmayor.com/word_vba_examples.htm of inserting
a sliding scale in a protected form based on the result of a dropdown field.
It would not be too difficult to base it on the result on an input box or
bookmark etc.

Thanks Graham. That code looks good for other templates that I'll need to
create but it's not exactly what I'm after for this template.

People need to use the scale to select their rating, then another person
selects their rating and an average is calculated in a text box. My boss
thought it would be easier for people to use a sliding scale to select their
rating, as most people respond to visual effects more than dropdown lists.

The template is a "Staff Performance Review".

I know of no way that you can insert such a scale in a freely editable
document - and even with the method used on my web site you need macros
either to prompt for the numeric information and to format the table used to
present the scale. Another method would be to use 5 exclusive check boxes in
a frame, but that would also require the form to be protected
