Smart Tags to Schedule Appointment automatically in Outlook

I have an appointment form that I would like to be able to select the field
type in the date and it automatically schedule the appointment to specific
calendar in Outlook.

Can this be done with a Smart tag or is there another way I can do this?

Any help would be appreciated
