I had this issue start and it turns out my ISP blocked all SMTP commands except their own. So all outgoing mail had to be sent via their SMTP server if you were on their "cable".
Most times an ISP will enact this policy and never tell their clients until you call and get a tech who knows about it.
I know cox does this not sure who else. On my home account even for my co-lo web/mail server I have to still use the cox smtp for out going mail.
Now at the office, we use a business line via cox and we can use or own smtp server for outgoing.
It is one place to look at if toggling AV doesn't work.
