specificying where a merged line came from

Hi there,

I saw one sample form where after two forms had been merged there was text
in front of the merged lines specifying where that line came from. For
instance lets say the item fields were merged and Jack put rabbit and Jane
put dog, in the merged form it would say:

Items
[Jack] rabbit
[Jane] dog

How do you set it up so that the person's name appears on the merged form,
mine right now just merges the data without any specificiation of where it
came from. Also, can it only pu the Name there or can you specify what field
to use as the identifying criteria?
