Speech dictation errors

When I finish a sentence or a phrase, or have just paused speaking, the words
"and" or "when" appear in the text. sometimes the will appear two or three
times in a row while nothing is being dictated or typed.

My microphone and speech training seem to be quite accurate and the program
works with high accuracy other than this problem.

Any suggestions on how to stop this random word insertion?

Which software are you using for dictation?
