Strange problem with Outlook 2007

I'm getting messages from people who I mail everyday and who are on my safe sender and safe recipient email address yet randomly outlook will recieve the messgae as {Spam?} then the message title.

Its very embarassing when I miss this and reply back to me my contacts with it on.

I've checked everything and turned the setting as low as possible, I've even disabled my 3rd party spam filer (bitdefender) but it still occurs, anyone have any Ideas its driving me in insane.
