styles and formatting

Everytime I open a new document in MS Office 2003 Word, the blank new
document page displays a picture in a picture box at the top of the document.
At the same time the "Styles and Formatting" box displays an unknown styles
name "Normal+Top". Before I can begin using the new document page I must
delete the picture box. I have tried everything I can think of, including
modifying "Styles and Formatting" to correct this problem. Nothing has
worked. Whenever I open Word the picture in a picture box is always
displayed.
PLEASE HELP. I AM DESPERATE!!
