Sub Form or second form

Hi there,

I am creating a call management system database and have
the following 2 forms.

Form 1 - Call Details
Form 2 - Third Party support Companies

On Form 1 (Call Details) - I have a check box to indicate
wether the call has to be dealt with by a third party
support company.

Therefore, if i tick the check box i would llke the third
party (form 2) to appear at the right hand side of
original (Call Details) form one or even as a new form,
Doesn't really matter, just as long as i can enter the
third party support company details.

ANy help on this matter vey much appreciated.
