Subform Header

II have a report that is pulling subform information from a form, I only one
the form header to appear once in the report. Right now every time it pulls
in the information it adds the header to each line. How do I set up the form
to only show it once with the information below?

Instead of "pulling in" the form info into yhe report's
detail section, do it in the report header section.
