Submit button doesn't work, neither does the confirmation page!

Hello everyone,
I am new to this forum and would very much appreciate everyone's advice
on my problem(s). I have created a form, located here
(http://www.uniquehorizons.org/form.htm) though I cannot figure out how
to get the submit button to work. I am trying to get it to email me the
results but when I edit the form properties, I get the "this form is
being created on a disk-based web..." and it forces me to remove the
email address. I can't seem to get it to input the results into a
database though I'll settle for either solution.

Also, after hitting the submit button, it doesn't go to the
confirmation page as I thought I had programmed it to do (found here:
http://www.uniquehorizons.org/form_confirmation.htm). Any thoughts???

Check the path of your save results file
You have
.../_private/form-results.txt
(one level too high)
and it should be
_private/form-results.txt
