"Submit" button quit working!

I designed our website (www.equinedentistryschool.com) in
MS Publisher 2000. I have NO instructions at all and
fumbled through with the wizard and trial and error.
After I uploaded my last revision my Student
Application "Submit" button (form control) quit working.
It says "page not available."

I referred to the link you recommended and could not find
an answer. As I said before, it used to work. Have I
deleted a page? I can't even remember how I set up the
original "form" when I was designing it and all I can find
on help does not tell "how" to assign labels. I don't
even know what terms to search for in the help!

On the FAQ page I state that form processing requires FP extensions AND http
publishing.
If your form processing is not working that would be due to one or both of
those requirements not being met.
If you require more documentation on forms processing I refer you to the
article on my sites 2000 page that covers "publisher and FP extensions".
