How do I insert a form so visitors of my site can submit their email
addresses to me for information updates?

Just go to Insert Form, and this will get you started. Now you can enter the
inputs and right click on the form to go to Form Properties and enter your
email address

You *must* publish this to a Frontpage website and not use FTP
