survey database

I am in need of a database to collect user demographics and survey questions
& results. I have seen that many people recommend the 'at your survey'
example by Duane Hookum, but I cannot download the database - I get an error
message 'not authorized to view this page' after I click the download button.
Does anyone know of anywhere else I can get a copy, or is there any other
example database I could look at?
