Survey form in Outlook

Hello everyone.

I have a .DOT form in Word with drop-down fields that I
previously used for a survey. I would like to do
something similar in Outlook.

How can I easily create a survey form in Outlook with
drop-down fields? I have searchd all over this
discussion board, and through the Outlook Help, but have
come up with nothing. Any help would be greatly
appreciated!
