Survey form not working

I need to submit a survey (exch 2000/Outlook 2000) for a 1,000 users. I
would like to know what's the most effective way to do this.

I go to
http://www.outlookcode.com/d/forms/survey.htm

and I download survey.zip.
I open the the template OK, however I click "Add" button and I can add no
question. That's Outlook 2000.

Any ideas on how to get this working ?

note: I posted this question on exchange newsgroup too.

You must publish the form before it will work.

Interesting:

I open the survey.ot.

I go to "Tools", Forms, "Publish" form as "Mytest". Organizational Forms
Library.

Then I click OK and get message

"Unable to successfully publish the form due to a MAPI error. The form
cannot be installed because you do not have owner permissions".
Why is that ? This is Outlook 2002.

On the surface, it may mean exactly what it says-that you don't have the
right to create new items in Org Forms.
