Survey form

1) Can I send, rtf, a simple custom survey form, no code just custom fields
e.g. y/n buttons, on an internal network, definition attached, that all
recipients can read and respond to without publishing on Public or
organizational folder?

2) I find the form I have developed arrives at recipients without
displaying, or when sometimes displayed, the reply creates a standard form.
I have identified the same Custom Form as the form for reply. Maybe this
means it searches for it in some published format, not as an attached form
definition. Even the Sample survey form, with code, at the very informative
and helpful Slipstick.com site does not display when emailed internally.

You would have to disable the reply and tell users to use Forward instead. A
reply will not use a custom form unless it's published.

published to work.
