survey maker for Outlook

Is there some convenient and simple survey maker for Outlook (sort of
add-ins or template), which allow create simple survey in Outlook html email
message, send it in internet and get results back? User makes the choice and
clicks on 'Send' button inside html email message. Is there other ways?

You might need to have an exchange server for this feature to work.
