I have an opinion survey containing Check Boxes and Text Form Fields within a
table that I prepared in MS Word and copied to an Outlook e-mail. The Check
Boxes and Text form Fields disappear or fall out before the intended
recipient gets the survey by Internet e-mail. I tried converting the survey
into a template thinking it would somehow do a better job of carrying the
formatting with it, buth this was not successful. I want the form to be able
to filled out on the original e-mail and the person filling it out only to
have to click on 'Reply,' to have it return to me. (compare to a simplified
version of Zoomerang or Survey Monkey). I am looking for help on how to keep
the Text Form Fields and Check Boxes.
