Survey prep'd & formatted in MSWord copied to Outlook loses format

I have an opinion survey containing Check Boxes and Text Form Fields within a
table that I prepared in MS Word and copied to an Outlook e-mail. The Check
Boxes and Text form Fields disappear or fall out before the intended
recipient gets the survey by Internet e-mail. I tried converting the survey
into a template thinking it would somehow do a better job of carrying the
formatting with it, buth this was not successful. I want the form to be able
to filled out on the original e-mail and the person filling it out only to
have to click on 'Reply,' to have it return to me. (compare to a simplified
version of Zoomerang or Survey Monkey). I am looking for help on how to keep
the Text Form Fields and Check Boxes.

You'd need to send the Word document as an attachment, not incorporate it into the body of the email message.

What Office version are you using?

MS Word and Office Outlook 2003 on Microsoft XP Professional SP2 (licensed
to a hospital). Would Mail Merge work?
If I opt to make the survey an attachment, I am concerned some of my
respondents will not fill it out. This is a 9 row, 3 column table that I'm
sending to physicians and nurse managers, that one "style" of survey they may
respond to, another they will ignore. Hence, the hope of keeping it inside
the original e-mail.

Mail merge won't produce email messages with fillable text and check boxes. Have you thought about using one of the web-based survey services? Those produce surveys that are accessible to the widest variety of potential respondents.

Yes, we have considered Zoomerang, though it is not our option of choice.
This is not only because of the cost, but if the survey worked as we hope, it
could be a useful tool in the future.
Is this not an option with the software version mentioned below?
I sent an email to my exchange administrator requesting permission to
publish forms to the Organizational Forms Library that you mentioned in
another discussion string. Would the assignment/permission facilitate my
goal?

If you can publish a form to the Organizational Forms library, then you can use an Outlook custom form to conduct surveys. That means starting over, as you can't turn a Word document into an Outlook form. You could consider using the generic survey form from http://www.outlookcode.com/article.aspx?id=63
