Survey / Questionnaire Database

Good Afternoon Everyone,

I have created the following tables for the data needed for my questionnaire:

tblRespondent

tblQuestions

tblAnswers

tblResponses

I have created the all relationships and completed all data entry. I am now
stuck on how I create the form.

My question is am I able to link more than one table to the form (not
subforms) or will I need to create a table that combines some of the data
from each table above? Please help me to understand what the best option
would be. If any of you have a template that would be awesome.

Will this work for Access 2003 or 2007? I am creating this database in the
2003 version but everyone will be updated to 2007 soon.

The sample database is A2000 so you should have no trouble
running it in later versions. However, you don't necessarily
need to use his sample application, you can just download it
to get ideas and methods you can use to finalize the application
you have already started.
