survey tables

[i am lost in the sauce]

hello all,

i am constructing a survey for my organization. i have looked at Mr.
Hookum's AYS.dbm. I am not sure how to apply what i am doing to that concept
he has presented. so i am going to lay out the basic info and see if you
guys or gals can help me get a design going.

i have 600-800 that are spread out at 14 different nursing homes etc. i have
the question i want to use in the servey roughly 20-30, most of them are
going to have the some response like a 1-5 rating some may be a little more
in detail after we edit some of them out. i need to be able to email this to
each of the users which are set up in our domain or network.

i know that i am really generalizing the scenario, but that is the basic
idea of what i am trying to do. if anyone needs more info in helping me
determine my database design i will answer them the best i can to fill in the
blanks.

At Your Survey isn't designed to email out and cull results from returned
emails. You could probably create a report that could be emailed but it
wouldn't be interactive.

You can search google on "online survey applications" for lots of options.
