I've created one via database utilities, switchboard manager, but can't
view it in forms. Normally, when one has been created the switchboard
can be accessed through forms.

Your problem was already posted earlier on September 24th by Dawn Shaferly. If you have not found a solution yet, you can see the solution I discovered with her thread. Please check existing threads before starting new ones
