Sync Issues - Local Failure

I have a custom form (based on the post form) that is used in the contact
folder. I have an instance where on an Exchange based Outlook 2003 client
that when I create this custom form in the contact folder it shows up in the
contact folder for a few seconds then gets moved to the Sync Issues - Local
Failure folder.

This is the same form that I use on every other machine (pst and Exchange
based) and it works fine.

What would cause this behavior

Hi

Have you tried to open the form when the problem occurred or once your
create it and it occurred.?
Have you tried a very simple form, e.g. there is nothing on the form to see
if we can reproduce the problem.
If yes, I suggest you recreate the outlook profile or create another new
one to see if that works.

I can open the form when it is in the local failure folder, however when I
move it back to the contacts folder it just gets sent back to the local
failure folder.

Have you tried my suggestion that create a simple test form?
Did that work for you?
Also have you tried with another new profile?
