Table design for a recurring monthly charges to clients

Any ideas on setting up table(s) for recording monthly charges incurred by
clients? Ideally, there would be different types of charges, and the charge
amounts would vary each month. Also would need to record payments, either
partial or full, with date.

Are there any templates available?
