Table of contents does not display the number for a specific heading

I am experiencing the following issue in one of my templates:

The table of contetns does not show the numbering of the TOC 3, but
only at a specific heading. All other TOC 3 are ok, as below:

2.5. heading 2....1
2.5.1. first heading 3....3
2.5.2. second heading 3....9
2.5.3. third heading 3...12
2.5.4. fourth heading 3...14
fifth heading 3...........17
2.6. heading 2...21

I've tried also with a similar heading in the document and made all
previous and following paragraphs the same to each other as follows:

....some Normal text
Figure 10. some figure and its caption with a Caption Figure style
<page break>
2.5.4. fourth heading 3
.... some Normal text
Figure 11. some figure and its caption with a Caption Figure style
<page break>
2.5.5. fifth heading 3
.... some Normal text

but still I can not see the number 2.5.5. on the TOC, eventhough
everything is the same for both heading 3's.

What could be the cause of this? And how can I resolve it?

Well, I figured after a more detailed search.
Sorry that I increased the signal to noise ratio in the group.
The problem was that I had a text box object which was anchored to
that specific heading. After removing the anchor to its appropriate
paragraph, all worked fine.
