Table of contents glitch

Wow, who knew :)

(duplicate entry...I realized that my title was too broad in the
original posting so I reposted with a better title...sorry about that)

My table of contents is generating a double line for one entry
only...the rest is fine. The "list of Tables" shows up twice. I've
deleted it completely (both lines disappear from the TOC); inserted a
new one, and I get two TOC entries for that list again.


Ideas?
