Table of Contents - how to add Headers into it

I have created a user manual at work. I'm all done except
formatting the table of contents. I figured out how to do
that, but I can't get the tool to add the "header" field as
the first sorting field in the table of contents. It
should be:

Header
Title
Subtitle

But only the Title & Subtitle appear. I went to help and
that didn't help at all. It told me to use the
Cross-reference tool in Insert, but when I do that, it
doesn't show the header at all. I have done everything it
told me to with no avail. Help!

Tables of contents by default include all paragraphs formatted with
*heading* (not header) styles.
