Table of Contents Not Saving Page Numbers Correctly

I am proofing, formatting, and editing a book for a
doctor. I have the master document set to automatically
number new pages. When I update the TOC the page numbers
are correct. I save and close the document when I am
through for the day. When I re-open the document, the
numbers will be correct sometimes, but sometimes every
page number will be 4.

Please help ASAP. We will soon send to the publisher, as
I am through with the book.
