Table of Contents repeating entries

I'm creating a ToC in Word 2002 based on styles I
created. The ToC will generate fine for a while, but then
one or more of the entries will repeat (e.g., Ch.1, Ch.
2, Ch.3, Ch.3, Ch.4). Any ideas on what's causing this or
how to fix the repeats?

You have manual page breaks. Delete them and instead format the paragraph
that you want to have a new page so that it has page break before. This is
paragraph formatting.

Thanks! The only problem I had with your solution was where I had inserted a page break because I wanted to put a full page graphic on the next page. Adding hard returns to get to the next page seemed to work, however

You can format the heading as "Page break before" without encountering this
problem.
